Meet with a Geriatric Case Manager at the Senior Center

 Support for Family Caregivers of Frail Adults

Sponsored by the Sinai Adult Day Health Program in conjunction with the Stoughton Council on Aging, this program provides caregivers the opportunity to speak one-on-one with a geriatric specialist nurse during a free 30 minute session.

The sessions are held on the 2nd Tuesday of the month between at the Stoughton Council on Aging.
